回顧全民健保推行當時，因為整合了多元分岐的體制，提升了公平與效率；也因為單一體制實施總額支付制度，控制醫療費用的上漲，開辦以來只上調過兩次保險費率，可說是傲視全球的台灣奇蹟。我們寄望未來如果能調整長照服務的支應方式，並整合健康保險與長照保險，加速落實以「人」為中心的整合照護，必能創造下一個奇蹟。 台灣自1995年開始推行全民健保，至今已近30個年頭。面對人口老齡化、少子化之下社區發展的困境，本書以既有的健保體制，來構思未來社區中應有照護服務的想像，探討社區中應有照護服務所耗費的資金與人力，也聚焦討論糖尿病等慢性病的社區管理、慢性精神病人照顧者服務及長期照顧，並以德國與日本長照保險的經驗為他山之石，總結實踐社區照護服務的建議策略。
When the National Health Insurance (NHI) was implemented in Taiwan, it successfully integrated a diverse and fragmented system, improving both fairness and efficiency. Moreover, the single-payer system, which adopted a global budget payment method, effectively controlled the rise in healthcare costs. Since its launch, the premium rate has only been adjusted twice, making it a global success story often referred to as the "Taiwan Miracle." Looking ahead, we hope that by adjusting the funding mechanism for long-term care services and integrating health insurance with long-term care insurance, Taiwan can accelerate the realization of person-centered integrated care, creating the next miracle.
